As an APTA Utah member, we are happy to support your efforts to market the ChoosePT campaign. 

Please provide your description of the event, how you plan to use the marketing materials, and how many you are requesting. 

In order to qualify for the marketing items you must meet all of the below:
- Active member of APTA Utah
- Participating in an upcoming event that will benefit the Physical Therapy profession
- Return unused items by coordinating with Katie
